Complete all required fields in the application.
In selected items in the application form, hover the mouse over the field to display hints, a brief description of the field and its format.
The application forms contains an “ADDITIONAL INFORMATION” field at the bottom. It is a free-text field which the Issuer may use to provide additional non-standard information or clarifications concerning the application.
After completing the application but before saving or printing it, we recommend that you check the information entered. To generate the application, click the blue button “Generate application”.
If any required fields are left void, they will be highlighted in red when you click “Generate application”. The application cannot be processed unless such fields are completed.
After checking information in the application and completing all required fields, you can save the application on any media and print it.
NOTE: If you click “RESET”, all data entered in the application will be lost.
Please be advised that applications with the required attachments or other documents submitted to GPW, signed by duly authorised representatives of the issuer, may be submitted in hardcopy bearing handwritten signatures or electronically in PDF format bearing qualified electronic signatures or trusted signatures.
Applications with the required attachments or other documents submitted to GPW are deemed delivered:
in hardcopy, when the counterpart is received at the Exchange Secretariat (address of GPW S.A: ul. Książęca 4, 00-498 Warsaw, Poland).
electronically, when delivered to one of the following e-mail addresses: gpw@gpw.pl , emitenci@gpw.pl.

Examination entries should be made by e-mail in PDF format (bearing a qualified electronic signature or a trusted signature), or in paper form (bearing a hand-written signature) no later than 10 days before the scheduled date of an examination.
